An opportunity has arisen for a highly motivated individual to join the Adult and Older People Team in a new and exciting development as the Intermediate Support Worker (Days) in the Transitional Care Hub within the Blantyre Care Campus.

The successful candidate will assist the Care Facility Team Leader (registered manager) and Intermediate Senior Support Worker with the day-to-day operation of the Transitional Care Hub. You will report to the Intermediate Senior Support Worker within the South Lanarkshire Health and Social Care Partnership, and meet all regulations as set out by the Care Inspectorate. You will contribute towards achieving individuals’ outcomes in line with the Health and Social Care Standards and provide the right level of support for people to enable them to live independently in their own homes.

As part of the role, you will be expected to:

  • Promote and support service users to achieve as much independence as possible in order that they can return to live as independently as possible, in their own home.
  • assess, plan, implement and modify interventions to meet the individuals’ needs
  • Undertake health screening protocols to ensure risks are identified and mitigated
  • Develop, implement and review person centred support plans including risk assessment
  • Assist with the admission, transfer and discharge of people to and from the Service
  • Assist individuals with mobility, including physical tasks using appropriate equipment, whilst following therapist instruction and guidance when required
  • Assist in maintaining and promoting sufficient nutritional intake and fluid balance,
  • Provide extended personal care within a supervised environment
  • Supervise individuals or assist with the administration of prescribed medications, undertake medication risk assessments and work with the individual to achieve independence in this area where possible
  • support Senior staff in completion of reports for case discussions as required

Hours: 36 per week
