Job Description

The posts are part-time the salary will be pro-rata to the number of hours worked.

The post holders will assist the Care Co-ordinator in the provision, development and effective delivery of care services for older adults. They will work as members of a committed staff team, providing a high standard of care and support to individuals and groups consistent with their needs and wishes. The post holders will be responsible for carrying out the day-to-day activities of the care home, delivering consistent high standards within the principles of person centred care.

You will provide personal care and support to residents as directed by senior staff, taking direction from the Care Coordinators. This will include integrated partnership working with NHS and local authority colleagues, the private and voluntary sector to provide a holistic approach to supports for older adults. The post holders will be directed by the Care-Co-ordinator and will liaise with residents, their family, friends, and community healthcare specialists to deliver care and related services to older people and those living with dementia.

The post holders will be responsible for carrying out assessments, compiling individualised care and support plans and conducting regular reviews of the care/support and progressive development of the care plans to ensure that the individual, changing needs of each person are fully met.



Requirements

This post is considered a Regulated Role with Adults under the Protection of Vulnerable Groups (Scotland) Act 2020 and as such membership with the PVG scheme is essential. Further details of this scheme can be found here.

Preferred applicants will be required to become members of the relevant PVG scheme, or undergo a PVG Scheme update check prior to a formal offer being made by West Dunbartonshire Council.

Responsibilities
