Job Description
This vacancy is for a 17.5 hour post.
As this is a part-time post the salary will be pro-rata to the hours worked.
This is an exciting opportunity to join West Dunbartonshire Health and Social Care Partnership’s Hospital Discharge team. The teams within the HSCP are fully integrated and consists of social work and health staff. The service focus is supporting individuals to have a seamless journey from hospital to return safely back home, whilst promoting individuals health and well-being and independence through a co-ordinated multi- disciplinary / agency approach.
The post holder will provide an assessment and care management service to individuals whilst they are in hospital, to be carried out within the framework of legislation, relevant policies and procedures. The successful candidate will have a strong sense of professional identity and an ability to work both independently and as part of an integrated team.
We are looking to recruit a dynamic, motivated, forward thinking individual who is looking for a new challenge and has the ability to work in a fast pace environment.
The post holder will manage and co-ordinate the delivery and development of Integrated Health and Social Work services with other service leads and integrated operations manager. Substantial post qualifying experience as a Social Worker is essential along with a robust knowledge of current relevant legislation. Experience of working within an integrated team is also desirable.
Requirements
This post is considered as Regulated Work with Adults under the Protection of Vulnerable Groups (Scotland) Act 2007 and as such membership with the PVG scheme is essential. Further details of this scheme can be found here.
Preferred applicants will be required to become members of the relevant PVG scheme, or undergo a PVG Scheme update check prior to a formal offer being made by West Dunbartonshire Council.
Responsibilities
