Job Description
An opportunity has arisen within West Dunbartonshire’s Community Hospital Discharge Team for two enthusiastic Social Workers to join a hybrid social care service supporting patients in the acute wards to discharge safely from hospital. This includes assessing patients to be considered for discharge home to assess within the community process along with assessing for long term care through the My Life Assessment process.
This is an exciting opportunity to work in a fast-paced, multidisciplinary environment where you will support adults in achieving safe and timely discharges from hospital and rehab settings. Your role will promote independence, well-being and person-centred care.
You must be a qualified Social Worker registered with SSSC and you will manage a caseload, completing My Life Assessments, feeding into AWI Case Conferences, Supporting the Single Point of Access Duty System and Adult Support and Protection Duty. You will be part of a multi-disciplinary team.
Requirements
This post is considered as Regulated Work with Children and Adults under the Protection of Vulnerable Groups (Scotland) Act 2020 and as such membership with the PVG scheme is essential. Further details of this scheme can be found here.
Preferred applicants will be required to become members of the relevant PVG scheme, or undergo a PVG Scheme update check prior to a formal offer being made by West Dunbartonshire Council.
Responsibilities
The employment for which you are applying is excepted from the provisions of section 4(2) of the Rehabilitation of Offenders (Exceptions) Order 1975. Applicants are, therefore, not statutorily entitled to withhold information about convictions which for other purposes are "spent" under the provisions of the Act. In the event of employment any failure to disclose such convictions could result in dismissal or disciplinary action.
