As a Social Care Practitioner at Limecroft Care Home, you will work as part of a team providing a high standard of care and support, in order to meet the personal, physical, social and emotional needs of 38 older people.

This position requires registration with the SSSC. The successful candidate will have six months from the date of commencement of this post to become registered with the SSSC. SSSC stipulate you must submit your application within three months of your start date. If you are already registered with SSSC you will be required to add West Lothian Council to your account.

This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.

It is essential that you possess an SVQ III in Health and Social Care or equivalent qualification. HNC is desirable.

You will provide direct personal care and support in accordance with individual care plans and contribute to the assessment, planning, delivery and review of care, to ensure residents’ assessed needs are met.

You will undertake key worker responsibilities for allocated residents and contribute to care planning and reviews, ensuring residents’ changing needs are identified and appropriately supported.

You will use the Access Care and Clinical electronic care planning and recording system to contribute to and update care plans and complete daily progress notes and other relevant care records.

You will administer and manage medication in accordance with Council policies, procedures and training, using the EMAR electronic medication administration system. This includes accurate recording of medication administration, medication ordering and receipt, monitoring for side effects and supporting residents who are able to self-medicate.

You will contribute to the care planning process and ensure that care practice is non-discriminatory and that residents’ personal beliefs, preferences, dignity and choices are appropriately acknowledged.

You will be aware of and responsive to the needs of older people with mental health problems, including dementia.

You will provide personal care and support with emotional needs of older people requiring residential care. This includes support with personal hygiene, dressing, mobility, eating and drinking, continence and maintaining relationships with family and friends.

You will work collaboratively with residents, families, carers, colleagues and other professionals to provide a person-centred service.

You will participate in handovers, reviews, staff meetings, supervision and annual development reviews and undertake training and development appropriate to the role.

You will provide a service compatible with professional social care practice and within the policies, procedures and standards set by the Service.

You should have an understanding of the specific needs of older people and the ability to communicate effectively with residents, relatives, colleagues and other professionals.

A shift pattern is in place, including weekends.

You will require to complete Disclosure Scotland/ PVG checks
