The Ability Centre Support Service is a day service provision for adults who have a physical disability or neurological condition. The service provides both a centre based and community outreach-based service. The successful candidate will be expected to work as part of an established staff team to support service users to be as independent as possible and where possible integrated in the local community.

Key Tasks and Responsibilities -
To facilitate a range of 1:1 or group activities either within the centre or in outreach settings which will encourage maximum independence and ensure involvement in local communities.
To facilitate service user placement reviews and liaise with a range of practitioners as required.
Complete reports for the review. Through this review processes identify individual service user progress and identify potential for accessing different service opportunities.
To act as a key worker to an agreed number of service users and develop, monitor and review care & support plans as required.
To work with a range of other professionals, students and volunteers and where appropriate offer support and supervision when they are working with service users
The successful applicant will be required to have –
Minimum qualification HNC Social Care and SVQ Level 3 or equivalent.
Ability to communicate effectively both verbally and in writing.
The post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme and undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
