Support Worker - Discharge to Assess

Location: South Ayrshire with travel around the area

Salary: £24,479 per annum based on 35 hours per week, pro-rata for roles under 35 hours (£13.45 per hour)

Hours: 18, 24 & 35 hours available

Contract: Permanent

Additional Requirement: Successful candidates will be required to register with the Scottish Social Security Council (SSSC) within 3 months of starting.

As a Support Worker, you’ll be at the heart of our regulated service, responding to referrals from health professionals and providing practical and emotional support to vulnerable adults. You’ll help prevent hospital re-admissions and promote wellbeing by enabling individuals to regain confidence and independence. What will a day in the life of a Support Worker involve?

  • Visiting service users in their homes to assess needs and create tailored support plans.
  • Assisting with daily living activities such as bathing, dressing, and meal preparation.
  • Liaising with health, housing, and social care professionals to ensure consistent support.
  • Carrying out welfare checks and helping individuals rebuild their independence.

To be a successful Support Worker, what will you need?

  • Experience supporting people with physical, sensory, or mental health needs.
  • Strong communication skills and the ability to work both independently and in a team.
  • Knowledge of NHS and social care services, and how to access them.
  • A flexible approach to working hours, including evenings and weekends.
