Community Navigator Job Description

We are looking for an excellent communicator and listener who can think creatively to solve problems and manage their workload effectively. The successful candidate will oversee a caseload of individuals and confidently prepare and present information to a diverse range of stakeholders. This role provides an opportunity to go beyond the scope of the work already being carried out with our core service SDS Forth Valley and our Supporting in the Right Direction Project by extending the current remit and enhancing the possibilities for people to experience a whole system approach to both universal services and statutory funded support.

Job Purpose

Working predominately in the Clackmannanshire rural areas the Community Navigator will support individuals in accessing the services, resources, and opportunities they need to improve their wellbeing. Working closely with health, social care, and community organisations, the postholder will provide person‑centred guidance, help people overcome barriers, and empower them to build confidence, independence, and connection within their community over all aspects of the lives.

The Community Navigator role is a Scottish Government funded initiative and some liaison and reporting with other associated partners will be necessary.

  • Maintain accurate and confidential records using agreed client recording systems.
  • Monitor progress, gather feedback, and contribute to evaluation reports.

  • Identify and respond appropriately to safeguarding concerns in line with local policies.
