We are looking for individuals to join our Home Assessment Team, putting people first and ensuring they get access to the right supports when they are ready to return home from hospital care. As a key member of the team, you will work closely with people, their families and colleagues across Health & Social Care to ensure we deliver good quality support in those first days after they return home, improving both their health and wellbeing and enabling them to live safely within their own homes.

As a good communicator, you must also be able to observe and respond to changes in the health and wellbeing of people in your care, updating and recording individual care plans as appropriate. Previous relevant experience working within a care setting and undertaking moving and assisting is desirable together with some IT skills and a current driving licence.
