Job Description
NLC7 £30,467 - £33,142 Pro Rata
Based in one of our locality support service teams, you will be a member of a team delivering high quality support/care packages to adults with a range of support needs, using person centred approaches in individual or small group settings. You will also be required to facilitate meaningful opportunities in mainstream community environments by accessing a range of activities which encompass social, leisure, lifelong learning and employment pursuits. In addition, you will lead on and contribute to the assessment, implementation, development and monitoring of individual support plans ensuring they continue to meet the needs of service users, including arrangements for measuring outcomes for and with individuals.
Educated to SVQ Level 2 in Social Care with a willingness to work towards SVQ level 3 within a defined timescale is essential. In addition, you will have experience of working with people with a range of support needs and of supporting individuals personal care needs. You should also have the ability to complete / contribute to assessments and care planning.
You will require to become a Protecting Vulnerable Groups (PVG) Scheme member prior to commencing in this role.
Work Pattern: Working flexibly 5 over 7 days and evenings and weekends as required.

*For all casual/sessional roles, you will receive 12.1% of your hourly rate for every hour worked in total recompense for annual leave, including public holidays. These rates will be indicated separately on your payslip. You will only be paid for the hours that you work.
