An exciting opportunity has arisen for a Social Worker to join the Care Home Assessment and Review Team (CHART). The Team work with Care Homes across the Clacks and Stirling HSCP supporting them to ensure standards of care and support for residents is at a level where quality of care through quality of service is provided.
As well as undertaking the statutory responsibilities, you will also be working within a framework to support Care Homes to maintain and improve standards of practice in line with National Care Home Contract and the Health and Social Care Standards - My support my life.
